This workshop will assist solo family doctors and family doctors working in group practice to identify their complex patients and appropriately refer them to local and provincial resources. They also will learn in this interactive workshop how to identify resources for their patients and activate them to reduce the burden on patient, family and healthcare system.
Identify patients who would benefit from improved coordination of care.
Describe the benefits of collaborative case management for complex patients.
Classify and describe existing resources for complex patients in your community and access and activate community resources for your patients.
