Following graduation from the University of Toronto in 1973; Dr. Kozak began his journey in family medicine. He opened his independent practice in 1975 and in 1998 he received a Fellowship in Family Medicine.

His professional experience includes 37 years in independent practice as a Family Physician. His practice continues to grow in generations with the provision of care extending from well baby care to the aging population.
Since the early 1980's Dr. Kozak has been the Medical Director for both the Ivan Franco Home and the West Park Hospital- Occupational Health Unit. He is also a courtesy staff member in Family Medicine at the University Health Network.

Dr. Kozak is continuously involved in additional professional activities. He has dedicated a lot of time as Principal Investigator to various phase II-IV research studies. He was the Co-Author for PEPWH Work and Health Policy Book as well as the Co-Author for the Agatha Study which was published in the Medical Journal, Nature.

Dr. Kozak speaks English, Ukrainian and German. He enjoys playing hockey and golf, hiking, carpentry and outdoor activities with his grandchildren.
